There are several tools available for use today that can lead us to root cause. The fishbone diagram, also known as Cause and Effect or Ishikawa diagrams, is a great tool to use for medium risk / medium complexity issues. When used correctly, it can lead the investigator and team to find the logical root cause(s) of your issues. Using the fishbone well, and including it in the documentation of your investigation, will be a key to satisfying the FDA’s expectation for thorough investigations. This webinar will add to those skills of a current investigator as well as equip someone new to this responsibility.
